Request for Special Accomodation

Requirement
The Certification Board of Nuclear Cardiology (CBNC), in accordance with the Americans With Disabilities Act (ADA, 1990), provides reasonable and appropriate accommodations for disabled individuals who supply appropriate documentation.

Applicants must request Special Accommodations at the time they make application to sit for the CBNC exam.

Document Content and Format
The applicant may submit a letter of request on appropriate letterhead outlining the reason for the request and the specific accommodation(s) needed. Alternatively, the applicant may submit the request on CBNC’s Special Accommodations Request Form.

Documentation from appropriate medical personnel must accompany the request. Additional documentation such as accommodations provided by other certifying boards may be included to further support the request.
